remote patient monitoring services, weight-loss programs, and any digital platform
operated by Quantara.
By accessing this site or receiving services, you agree to these Terms.
If you do not agree, please discontinue all use immediately.
1. Acceptance of Terms
By using this website or our telehealth services, you confirm that you:
Are 18 years of age or older
Are physically located in the state of Florida at the time of any telehealth appointment
Understand these Terms form a legally binding agreement
Consent to electronic communication and medical evaluation
2. Not for Emergency Use
Quantara does not provide urgent or emergency medical care.
If you are experiencing chest pain, difficulty breathing, severe symptoms, or believe you
may be experiencing an emergency:
Call 911 immediately or visit the nearest emergency department.
This website and our telehealth platform are not substitutes for emergency care.
3. Medical Services Provided
Quantara offers non-emergent clinical services, including:
Medical weight-loss management
GLP-1 agonist therapy (e.g., semaglutide, tirzepatide)
Phentermine-based weight loss therapy
HIV telemedicine care
Chronic disease management
Remote Patient Monitoring (RPM)
Preventive health and wellness services
Quantara may decline care that falls outside the scope of practice for an APRN under
Florida law.
4. Telehealth Services & Limitations
By using Quantara telehealth services, you agree that:
You must be physically located in Florida during your appointment
Your clinician may request an in-person evaluation before prescribing certain
medications
Technology issues may occur and do not constitute medical negligence
Telehealth is not appropriate for all conditions
We reserve the right to reschedule or redirect you for in-person care when clinically
necessary.
5. Prescribing & Medication Policies
Medications—including GLP-1 therapies, phentermine, antiretroviral HIV medications,
and chronic disease treatments—are prescribed only when clinically appropriate.
Prescription is not guaranteed. We may decline or discontinue medication for:
Safety risks or contraindications
Missing required labs or diagnostics
Concerning side effects
Misrepresentation of medical history
Non-compliance with care plan
Phentermine Requirements
You must provide:
An EKG dated within the past 30 days, OR
An EKG performed by Quantara’s mobile clinical service (if available)
6. Weight-Loss Program Participation
When you enroll in Quantara’s weight-loss program, you agree to:
Submit labs or EKGs when requested
Report symptoms or complications accurately
Attend scheduled follow-up visits
Comply with medication, monitoring, and lifestyle recommendations
Non-adherence may result in dismissal from the weight-loss program.
7. Remote Patient Monitoring (RPM) Program
Participation requires:
Signed consent
Daily/regular device use (BP cuff, scale, glucometer, etc.)
Accurate symptom reporting
Maintaining connectivity with your device(s)
RPM billing follows CMS/insurance guidelines. Non-compliance may result in removal
from the program.
Quantara is not responsible for:
Device malfunction
Connectivity issues
Delays caused by third-party vendors
8. Payments, Billing & Refunds
Self-Pay Services
Self-pay fees are due at the time of service unless otherwise arranged.
Insurance Billing
Insurance may be billed for:
Telehealth medical visits
HIV care
Chronic care management
RPM services
You remain financially responsible for:
Deductibles
Copayments
Non-covered services
Refund Policy
Due to the nature of clinical services, all services rendered are non-refundable.
9. Website Use & Restrictions
By accessing this website, you agree NOT to:
Use the site for unlawful purposes
Copy or distribute content without permission
Introduce viruses or harmful code
Attempt to access unauthorized systems
Misrepresent your identity
All content—including logos, branding, and educational material—is proprietary
intellectual property of Quantara.
10. Privacy & HIPAA Compliance
Quantara maintains HIPAA-compliant practices to protect your medical information.
However, you acknowledge that:
Electronic communication carries inherent risks
No system is 100% free from security breaches
Third-party vendors (such as RPM device companies) have their own privacy policies

11. Technology Requirements
Telehealth appointments require:
A smartphone, tablet, or computer
Stable internet connectivity
Functioning audio + video
Access to your patient portal
You are responsible for maintaining your own technology.
Quantara is not liable for lost appointments due to technical failures.
12. Limitation of Liability
To the fullest extent allowed by law:
Quantara Clinical & Diagnostic Center LLC is not liable for:
Damages caused by use or inability to use this website
Delays, disruptions, or errors in telehealth technology
Outcomes resulting from incomplete or inaccurate information provided by the patient
Interruptions caused by third-party vendors or service providers
Your use of our services is at your own risk.
13. Indemnification
By using our services, you agree to indemnify and hold Quantara harmless against any
claims arising from:
Misuse of this website
False health information
Violation of these Terms
Unlawful or harmful actions
